Nurses contributing to the development of transitional care practice for older adults

On Friday 8 October at 13:00, Rikke Buus Bøje defends her PhD dissertation entitled "Nurses' development of transitional care for older adults through expansive learning actions".

In a recently completed PhD project from Aarhus University and Randers Regional Hospital primary care and hospital nurses' contribution to the development of transitional care for older adults' was investigated.

The project has resulted in a new transitional care model emphasising person centred care. The model addresses contradictory values, needs and demands from patients and the nurses' respective organisations. In addition, the model supports nursing ideals and professionalism in transitional care.

Nurses with different aims for patient care are involved in transitional care for older adult. This can lead to fragmentation in patient pathways, to readmissions and severe events. Education of nurses is considered a means to improve transitional care. However, targeted education is limited. Therefore, PhD student Rikke Buus Bøje has developed a shared learning intervention for nurses from primary care and hospital to investigate how learning can support nurses to improve their practice.

Yet unpublished results concerning the type of learning that supports the development of transitional care and what contributes to the emergence of agency to develop will be presented and discussed at the defence.
